Program Description:  Let’s Move Holyoke 5-2-1-0 will improve weight status in low-income children and families in Holyoke, Massachusetts. The project will provide culturally appropriate, bilingual, healthy weight interventions including physical activity promotion and delivery of relevant messages around healthy eating. Let’s Move Holyoke 5-2-1-0 will significantly impact the obesity epidemic by implementing an evidence-based and tested model for individual and community engagement to promote healthy weight.

Projects Goals & Objectives:  The overall goal is to improve weight status in low-income, high-risk, children and families in Holyoke through the implementation of a multi-sector, bilingual intervention model that is integrated with primary care delivery, supported by healthy behavior change and embedded in the community.
